Official Membership Card: Request List

So, here it is. The official membership card of The Pack! Voted on by the members as the best design.
Do NOT send your info to the person making your card until they request it (sent via PM). Do NOT post your info publicly in this thread. We will not post it publicly, just in case you want to keep it private. We need the following...
-Pack Name (Basically your forum name.)
-Real Name (Your real first and last name. You can also choose to put "anonymous", or just your first name if you want.)
-Territory (Your state/territory/province and country. Does not include city.)
-Birthdate (Please don't lie about your age.)
-Member Since (It's listed in your forum profile.)
-Picture (Can be either your real photo, or an avitar of your choice. Make sure it is big enough to fit, or we'll have to stretch it out.)
Your membership card will not be posted by the card distributor staff on this forum for all to see. It will be sent to you in private, and we delete the hosted file once it is received. You may feel free to display it online at your own descretion. Once handed over to you, your card is your property to do whatever you want with. You are not getting a printed card mailed to you, so do not give out an address. I don't have the budget to do that. This is just the high-res bitmap file that is being sent. It is up to the individual member to print it out, laminate it, or back it with plastic. Your neighborhood Kinkos or print shop should be able to do it for you for a low price.
Requests are first come first serve via a post in THIS THREAD saying you want one. Again, do not PM the membership card distribution staff your card's information until it is requested of you. We don't want the card distributors bombarded with PMs.
